RISE is a collaborative program providing integrated services coordination and a range of supportive services to enable homeless individuals with disabilities access and establish housing. Disabilities may include alcohol/drug, psychiatric, physical, cognitive, sensory, and developmental. Each participant of the RISE program is assigned an Integrated Service Coordinator who assists clients through the range of services in a coordinated fashion. Services are provided on-site, by home visits, and in the community. Services include:
Emergency and Transitional Shelter
Alcohol and Drug Treatment
Mental Health Treatment/Support Services
Health Education and Treatment Advocacy
Life Skills Training
Housing Search Assistance
Vocational Assessment and Employment Services
Veteran Specific Services
