HOPE (Homeless Outreach People Empowerment)

The HOPE (Homeless Outreach People Empowerment) Project began in 1999 and operates as a collaborative between Abode Services, Tri-City Health Center, and the Cities of Fremont and Livermore. The Program operates out of a 37-ft, state-of-the-art, mobile health clinic to bring services to individuals and families who are homeless, many who are living on the streets.

The HOPE Project team travels to various sites throughout Southern and Eastern County (the cities of Fremont, Pleasanton, and Livermore), five days a week, and offers primary and mental health care, social services, and substance recovery services. The mobile health clinic has a medical exam room, complete with an EKG and pulmonary testing equipment, two private, sound-proof confidential counseling rooms, and an intake area. Additionally, there is an outdoor waiting area complete with awning to shelter clients waiting for services from rain and the hot sun during summer. All services are provided free of charge.

The HOPE Project staff is committed to "do whatever it takes" and "meeting clients where they are at." In addition to the services above, the HOPE Project team provides clean socks and underwear as well as tents, sleeping bags, jackets/hats to keep them warmer at night. The mobile clinic serves over 100 people per month and has been able to form long-term relationships that have resulted in the attainment of permanent housing, benefits acquisition, an ability to seek and obtain stable employment, and those severely addicted to substances to request placement in detox facilities and rehabilitation treatment centers.
