RISE
RISE is a collaborative program providing integrated services coordination and a range of social services to help homeless individuals with one or more disability find and maintain housing. Each participant of the RISE Program is assigned an Integrated Service Coordinator who assists them in accessing services in a coordinated fashion. Services are provided on-site, through home visits, and in the community. They include:
- Emergency and Transitional Shelter
- Alcohol and Drug Treatment
- Mental Health Treatment/Support Services
- Health Education and Treatment Advocacy
- Life Skills Training
- Housing Search Assistance
- Vocational Assessment and Employment Services
- Veteran Specific Services
